Realtime care gap closure No guesswork No delays

Meaningful care and coding insights integrated directly with your EMR

Bridges the analytics-clinical gap. We deliver prioritized risk and quality gaps directly into provider EMRs, inboxes, and/or point-of-care tools, making it easy for care teams to act, document, and close gaps during routine visits.

Overview

Even the most advanced analytics are only meaningful if acted upon quickly. Today, many health plans struggle to convert suspecting data, risk gaps, and quality opportunities into real-time, clinical action. Faxes, manual chart pull requests and/or non real-time portals don’t cut it. These lead to missed interventions, increased care gaps and reduced member outcomes.

Built with industry-standard interoperability and FHIR-based protocols, we integrate across EMR types, delivery networks, and provider group structures.

    • Suspect conditions
    • Missing supplemental data
  • Prioritize scoring to focus on the most impactful gaps
EMR-Triggered Alerts & Pre-Visit Prep
Invent2EMR

EMR-Triggered Alerts & Pre-Visit Prep

  • Surface care gaps prior to appointment or during patient check-in
  • Enables nurse line, rooming staff, or clinician to
    act during the visit
Bi-directional Feedback & Reconciliation
Invent2EMR

Bi-directional Feedback & Reconciliation

  • Track when gaps are reviewed, addressed, or deferred
  • Send structured gap resolution data back to Invent Health for downstream recapture and risk adjustment factor (RAF) updates
Provider Stratification & Performance Reporting
Invent2EMR

Provider Stratification & Performance Reporting

  • Monitor documentation effectiveness by provider or group
  • Identify high-performing groups or those needing coaching
Chart Data Integration
Invent2EMR

Chart Data Integration

  • Ensure seamless chart retrieval for accurate,
    timely chart reviews
  • Automated coding for risk and quality programs

Invent2EMR delivers real-time insights into EMRs and intelligently pulls chart data  to power risk validation, quality analytics, and clinical audits with the most current data available.

Invent Health retrieves structured and unstructured clinical data from provider systems, thus eliminating the need for manual chart pulls. This process enables automated access to encounter notes, lab results, and provider documentation for faster, more accurate analysis.

Invent2EMR

How It Works?

1. The Invent Health analytics platform generates member-level gaps (risk + quality + Annual Wellness Visit (AWV) and In-Home Assessment (IHA) targeting)

2. Structured payloads (FHIR, HL7, or batch SFTP) are delivered to the provider’s system

3. Providers receive easy-to-use summaries via their workflow or third-party app

4. Gap resolution and documentation are returned to Invent Health to track closure and ROI

Who uses it
Invent2EMR

Who Uses It

  • Health plans to engage their networks more effectively
  • Provider groups & IPAs participating in shared risk, value-based care (VBC) or RAF-linked contracts
  • ACO REACH, DSNP, and managed Medicaid plans needing high-volume provider integration
  • Managed Services Organizations (MSOs) and delegated entities with EMR access and performance accountability

Outcomes Delivered

Impact Area Traditional Approach With Invent Health Connectivity
Gap Closure Rates Delayed delivery = low closure (~20–30%) Real-time pre-visit push = Improve closure (50–60%+)
Provider Experience Manual portals, faxes, or spreadsheets EMR inbox / embedded gaps, fewer disruptions
Documentation Quality Gaps missed or not properly resolved Embedded NLP-supported summaries = better coding
Recapture Tracking No loop-back from provider Bidirectional resolution = real-time RAF tracking
Frequently asked

Getting risk and quality gaps into the EMR

How care and coding insight reaches the clinician during the visit, and what integration actually requires.

How do health plans get risk and quality gaps in front of providers at the point of care?

Most still do not. Faxes, manual chart pull requests, and non-real-time portals are the common channels, and each one arrives too late or in a form the clinician has to leave the chart to read. The result is missed interventions, increased care gaps, and reduced member outcomes.

Invent2EMR delivers prioritized risk and quality gaps directly into provider EMRs, inboxes, and point-of-care tools, so care teams can act, document, and close gaps during a routine visit rather than after it.

Delivery is only half of it. Provider-facing smart gap cards summarize suspect conditions and missing supplemental data with prioritized scoring, so the clinician sees the most impactful gaps rather than an undifferentiated list.

Which EMR integration standards are supported?

FHIR R4, HL7, SMART-on-FHIR, and API-based delivery, built to integrate across EMR types, delivery networks, and provider group structures.

Standards-based delivery is what makes this scale across a provider network. A plan with dozens of practices on different systems cannot build a custom integration per practice.

Does gap resolution flow back to the plan?

Yes, and the bidirectional half is what makes it a closed loop. The system tracks when gaps are reviewed, addressed, or deferred, and sends structured gap resolution data back to Invent Health for downstream recapture and RAF updates.

Without the return path a plan has delivered a gap list and has no idea what happened to it. With it, the plan knows what closed, what was deferred, and what to escalate.

Can we see which providers document well and which need support?

Yes. Provider stratification and performance reporting monitors documentation effectiveness by provider or group and identifies high-performing groups alongside those needing support.

Used well this is a coaching input rather than a scorecard. The groups that need support usually need a specific documentation habit changed, and knowing which one is what makes provider engagement land instead of abrade.

How does this reduce chart chase burden?

By pulling structured and unstructured chart data directly through the integration, which removes manual chart pull requests from the workflow.

Chart chase is the most abrasive part of the plan-provider relationship. Every retrieval request that becomes an automated data pull is one less administrative demand on a practice that has no incentive to prioritize it.
